Sports Medicine Center
The Lassen College Sports Medicine Center, located within the Sports Complex, is accessible through both the men's and women's locker rooms. This dedicated healthcare facility serves as the central hub for injury prevention, evaluation, treatment, and rehabilitation for all ten intercollegiate athletic programs.
At Lassen College, the health, wellness, and safety of our student-athletes are our highest priorities. The Sports Medicine program is led by a certified athletic trainer and supported by a team that includes a board-certified family medicine physician, the local hospital, and a regional orthopedic center. Together, this collaborative network ensures comprehensive, high-quality care for all Cougar student-athletes.
The Sports Medicine department is committed to continuous improvement through the integration of current best practices, emerging technologies, and ongoing professional development.
Pre-Participation Physical Examination Requirement
All prospective student-athletes are required to complete a pre-participation physical examination specific to athletic activity before engaging in any practices, conditioning, or competition.

This examination must be documented using the official Lassen College physical and must be completed and signed by a Medical Doctor (MD), Doctor of Osteopathy (DO), Physician Assistant (PA), or Nurse Practitioner. Physicals completed by Registered Nurse (RN), Chiropractor (DC), or Physical Therapist (PT) will not be accepted.
Returning Student-Athletes
Returning student-athletes are not required to submit a new physical examination, provided their previous physical is still valid and on file with the Sports Medicine Department. However, each year they must:
- Update and resubmit their Medical History on SportsWare Online
- Review, sign, and date a new Authorization to Treat form
- Ensure that all personal, emergency contact, and insurance information is current and accurate
These annual updates are essential to maintain eligibility and ensure appropriate medical care during the academic year. Athletic participation will not be permitted until all documentation is complete and approved by the athletic training staff.
Athlete Medical Records & Documentation – SportsWare Online
Lassen College utilizes SportsWare Online as the official electronic medical records system for all student-athletes. This secure, HIPAA-compliant platform allows athletic training staff to document injuries, treatments, and rehabilitation progress, while also providing a centralized location for medical forms and clearance documentation.
All incoming and returning student-athletes are required to create a SportsWare Online account. Through this portal, athletes will complete required pre-participation forms, upload their physical examination documentation, and update any relevant medical history. Timely and accurate completion of these records is essential to ensure eligibility for participation in athletics.
